Step 2: Prepare the Required Documents
To avoid delays, make sure to include:
- Receipts: Itemized receipts showing treatment costs and payments made
- Doctor Referrals: If applicable, include referral letters or prescriptions
- Test Results: Attach any lab results, X-rays, MRIs, etc.
